Family Caregiver Navigator is currently accepting participants on behalf of the Idaho Caregiver Alliance. The program provides free care plans, resource referral services and general caregiver support via zoom and/or telephone.
The Navigator first conducts an assessment of overall caregiver wellness, then together the caregiver and Navigator create a care plan to connect caregivers with critical support services throughout Idaho. These support services may include:
-Workshops and trainings
-Therapy or support groups
-Home aide assistance
-Financial support policies
Caregivers include any adult who cares for another person with a physical, intellectual, cognitive disability, chronic, or mental health condition(s). The person they care for can be of any age.
This research-based model is shown to reduce stress and burden on caregivers. The program aims to improve the quality of life for the caregiver and the person they care for. Take the short screener to receive your initial assessment results. A Navigator will then reach out to see if you’d like to complete the full assessment and receive a free care plan.
Professionals and providers can submit a referral of someone in need of caregiver support and our Navigators will attempt to make contact twice within 48 hours.
This is a pilot project currently restricted to SW Idaho. The program includes Ada, Adams, Boise, Canyon, Elmore, Gem, Owyhee, Payette, Valley, and Washington counties.
